What to Expect During Your First Visit
Your first appointment will last one hour. We will be discussing your health history, your goals and how you want us to help you. Your dietitian will evaluate and assess how to approach your specific nutrition/health concerns. Together we will identify how to get started with your change process and together we will learn from your challenges/set-backs and celebrate your successes! The process of change is a journey over a period of time which frequently resembles three steps forward and two steps back. Your success will be worth each moment of effort as you enjoy improved health and increased empowerment! Expect follow-up appointments, assignments to work on, and reports/communication with your doctor and/or counselors.

4200 W. Memorial Rd - Tues. 9am-5pm, Wed. 9am-5pm, Thurs. 9am-7pm, and one Sat/month
South Okla.City - Appointments can be scheduled on Wed. and Thurs. afternoon
Shawnee - Appointments can be scheduled on Friday
New Patient Forms
For your first visit please fill out all of the new patient forms and bring them to your first appointment. The forms may be found here.
Insurance Information
Your insurance coverage is an agreement between you and your insurance company. If it is important for you to have financial support from your insurance company, you need to call the toll free number on the back of your insurance card to verify your benefits for medical nutrition therapy (MNT). Many insurance companies do provide coverage for MNT, but it is different with each company's policy.
Cancellation Policy
When we schedule an appointment with you, we have very gladly reserved a specific block of time for you. We require a 24 hour notice of cancellation. We do not provide appointment reminder calls. Please keep your scheduled appointment because we look forward to seeing and working with you. If you cannot provide us with a minimum of 24 hour notice of cancellation, you will be charged for the time we reserved for you. This policy applies for your initial appointment and all follow-up appointments.
