Your personally
tailored, and flexible nutrition program includes:
- Review
of 4 page medical/nutrition history completed by you including at
least 3 days' intake records and list of your medications.
- Identification
of what food habits you are willing to change and those you don't
want to change.
- Determination
of your health/nutrition goals.
- Review
of medical/laboratory data from your physician.
- Analysis
of current intake pertinent to your diagnosis and goals, i.e., intake
of calories, fat grams, carbohydrates, protein, etc.
- Assessment
of your level of interest, motivation, degree of commitment to comply
with recommendations.
- Nutrition
approach is developed based on all information, all data/history collected
and communication with your physician to clarify any questions or
orders.
- One-on-one
coaching/counseling/educating.
- Follow-up
appointments to monitor progress, adjust plan as needed, keep you
motivated and focused on your goals.
- Reports
sent to your physician to keep him/her informed of your progress.
